Select the Service

From the Services menu, click on any existing service to open the management sheet.

Switch to Forms Tab

In the management sheet sidebar, click on Forms. This will display all consultation forms you've built in the Forms Manager.

Select and Save

Select the forms you want linked to this service and then click the Save Changes button at the top of the sheet.

Open the global menu and tap the Services tab.

Tap on any service to open its management sheet.

Tap the Forms option in the menu.

Select your forms and tap Save Changes at the bottom.
