What is a form?
A form is a structured document you send to clients or have technicians complete on-site. You build forms once in the Forms Manager and reuse them across as many services and appointments as you need.
Forms replace paper checklists, PDF agreements, and back-and-forth emails. Everything is captured digitally, signed where required, and stored against the appointment.
How forms are structured
Every form is made up of three layers. Understanding the hierarchy makes it much easier to plan your forms before you build them.
Form
The top level — has a name, configuration, and one or more sections
Sections
Groups of related fields — each assigned to a client, technician, or preset type
Fields
Individual questions or inputs — short answer, yes/no, checkboxes, drop-downs, and more
Three section types
Each form can contain a mix of three section types. The type controls who fills it and what fields are available.
Client details
One per formA preset section capturing name, email, mobile, date, and address. Fields automatically map to the client's CRM record — no manual entry needed.
Technician details
One per formA preset section completed on-site. Captures the technician's name, the job date, and the worksite address. Highlighted in blue in the form preview.
Custom section
UnlimitedFully configurable. Add a title, optional description, any combination of field types, and assign it to a client or technician.

Signature capture
Each form has two independent signature toggles you configure in the first step of the form builder. Both are optional — enable only what your workflow requires.
Client signature
Appended after the Client details section. The client draws or types their signature to confirm the information is accurate.
Technician signature
Appended after the Technician details section. The technician confirms they have reviewed the scope of work and the stated service terms.
Form configuration
When you create or edit a form, the first step is Configure — where you set up the form-level settings before building sections.
Form name
A required internal name for identifying the form in your Forms Manager and service settings.
Require client / technician signature
Two separate Yes/No questions. Both must be answered before you can proceed to the Build step.
Completion frequency
Choose whether the form triggers every time the service is booked, or only the first time per client.
Link to services
Optionally attach the form to one or more services so it triggers automatically on matching appointments.
Linking forms to services
Services are the types of work you offer — things like a drain inspection, CCTV survey, or a maintenance visit. Each service can have one or more forms attached to it. When a booking is confirmed for that service, the linked form is automatically sent to the client — no manual sending needed.
You can link a form in two places: during the Configure step inside the form builder, or afterwards by opening any service from the Services page and switching to its Forms tab. Either way, the connection is immediate.
Each form also has a Completion frequency — send it every time the service is booked, or only the first time a client books it. Use "every time" for job-specific checklists and "only once" for consent forms or onboarding questions.
